Resident #R294 was observed in his room sitting up in a wheelchair with elongated jagged fingernails with brown matter underneath. Interview with the certified nursing assistant assigned to the resident revealed that nail care had not been provided for approximately 14 days. Record review showed the most recent ADL care plan dated 2024-09-01 directed daily fingernail care.
Treatment / services to prevent / heal pressure ulcers
Tube feeding pump for Resident #R055 was running at the prescribed rate but the feed bag had not been changed within the 24-hour limit per manufacturer guidance. Bag change log showed last change 31 hours prior.
